At Dalworth Nursing & Rehabilitation Center our services benefit individuals whose activities of daily living cannot be adequately provided for in a home setting. These individuals may require a combination of skilled medical services and general nursing on either a short or long-term basis due to a prolonged acute illness or chronic disease. Our care management team works closely with your physician or our medical director to develop and implement a goal-oriented individualized plan of care.
