Our case management system promotes resident advocacy. The interdisciplinary team of caregivers provides sound management and recommendations regarding the resident's clinical and financial resources to positively impact quality of life.
The discharge planning process begins prior to admission to the home, at which time the resident and family discuss goals for recuperation and plans for on-going care. During care delivery, the plan is re-evaluated by all members of the team and the levels of care are adjusted according to resident progress and need. This care planning process is an integral part of our team's ability to ensure that goals are met as quickly as possible.
